Staying strong, steady and confident at home matters more as we get older. Our aged care physiotherapy focuses on the things that make everyday life easier — getting up from a chair, walking to the shops, managing steps, and reducing the risk of falls.

We visit older adults in their own homes and apartments, in retirement villages and in residential aged care. Families are welcome to be involved, and we keep GPs and care providers updated with your consent.

Care is led by Daniel Lee, an APA Titled Pain Physiotherapist experienced in aged care, falls prevention and persistent pain.

How we help older adults

Every program starts with a thorough assessment in your own environment, then a plan built around your goals.

  • Strength and balance training
  • Falls prevention and home safety review
  • Walking, transfers and getting up from the floor
  • Recovery after a fall, illness or hospital stay
  • Arthritis and persistent pain
  • Walking aid and equipment recommendations

Falls prevention

Falls are a common reason older people lose confidence and independence. A physiotherapy assessment looks at strength, balance, walking, footwear, medicines you've been told about by your GP, and hazards around the home. We then set up a practical exercise program and safety changes that may help reduce your risk.

Working with families, GPs and providers

We can talk with family members, your GP and your Support at Home provider or aged care team so everyone is working towards the same goals. With your consent, we send clear reports after the initial assessment and as your program progresses.

Funding

Support at Home (formerly Home Care Packages), Medicare GPCCMP, DVA and private clients are welcome. In residential aged care, physiotherapy is usually arranged with the facility — ask the care manager, or contact us and we'll help.

Frequently asked questions

Yes. We visit private homes and apartments, retirement villages and residential aged care across our Sydney service areas.

Yes — family members and carers are welcome, and it often helps everyone understand the exercise program and safety advice.

Physiotherapy can be arranged through your Support at Home provider when it meets your assessed needs. Participants make no contribution for clinical supports such as physiotherapy, but it does draw on your budget.

Why home visits

What we can see in your home that a clinic can’t

In a clinic, a physio sees how you move on a treatment bed. At home we see how you get out of your own bed, onto your own toilet and up your own front step — the places where everyday life, and most falls, actually happen. After five years of home visits, these are the things we check most often.

  • The chair and bed you actually use

A favourite lounge chair or bed that sits too low is one of the most common reasons standing up becomes hard. We practise on your own furniture and suggest simple changes.

  • The shower and bathroom

A step into the shower, a slippery floor or nothing to hold onto. We look at how you really get in and out, and whether a rail, shower chair or OT review would help.

  • Rugs, cords and clutter

Loose mats, trailing cords and tight spaces are easy to miss until you walk the route with someone. We walk it with you.

  • The trip to the toilet at night

Getting up in the dark, half-awake, is a high-risk moment for many older people. We check lighting, the path and how you get up from bed.

  • Your walking aid

Frames and sticks are often set at the wrong height — or left in the corner. We check the fit and practise using it where you need it.

  • Lifts, corridors and the car park

In apartments the hard part is often the walk from your door to the lift, the car park or the street. We practise the real route so you can keep getting out.

Persistent pain? Home is where we work on it

Our director, Daniel Lee, is an APA Titled Pain Physiotherapist with a Master of Medicine (Pain Management). With long-term pain the goal isn’t only less pain — it’s doing more with less worry. At home we can work on the exact task you’ve been avoiding, like hanging out the washing or getting into the car, and build it up step by step.

What to expect

  • We call you back within 24 hours of your enquiry.
  • Your first visit is usually within a week.
  • The first visit takes about 60 minutes — assessment, goals and a home program.
  • Reports go to your GP, coordinator or case manager as agreed with you.
  • Family and carers are welcome to join.
